diff-font-lock-syntax: clarify distinction between t and hunk-also

* lisp/vc/diff-mode.el (diff-font-lock-syntax): Rework docstring.
(diff-syntax-fontify-hunk): Never use the hunk method when
diff-font-lock-syntax is just t.
