semantic: Deal with missing prefix when completing symbol

* lisp/cedet/semantic/ia.el (semantic-ia-complete-symbol): Protect
 against a nil prefix.
 (semantic-ia-complete-symbol-menu): If bounds are empty (no prefix),
 don't delete region.
