Write autoloads file once only

* lisp/emacs-lisp/autoload.el (autoload-find-generated-file):
Simplify.  Don't bother about ensuring the output file exists.
(autoload-generated-file): Add doc.
(autoload-ensure-writable): Update doc.
(autoload-ensure-file-writeable): Handle non-existing file.
(autoload-ensure-default-file): Remove function.
